APPOINTMENTS
The River Grove Public Library is an official Passport Acceptance Facility and accepts on behalf of the U.S. Department of State ONLY applications for new passports and passport renewals for children under age 18. (DS-11 form).
Due to high demand, you MUST make an appointment. The library’s passport services are available by appointment only by calling 708-453-4484. No Walk-ins.
Library Passport Service Hours
Monday - Thursday: 10 am to 6 pm
Friday - Saturday: 11 am to 3 pm
Urgent Need: Only the Chicago State Department office can assist with urgent passport requests. You may qualify for an urgent appointment if you need to travel to a foreign country in the next two weeks because your immediate family member outside of the United States:
- Has died, or
- Is dying (hospice care), or
- Has a life-threatening illness or injury

Passport application (DS-11): Applications may be filled out online at travel.state.gov and then printed single-sided.
The library also provides blank applications which must be filled out in BLACK ink only. NO white-out. NO scribbles. Must be legible.
DO NOT SIGN THE APPLICATION UNTIL ASKED TO DO SO BY THE AGENT.

Evidence of Citizenship
- Fully Valid, undamaged US passport (may be expired)
- U.S. Birth Certificate **(MUST SHOW PARENT NAMES)**
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship
AND
Primary Photo ID:
- Valid Illinois State Driver's License
- US Government ID (Military ID)
- Valid Foreign Passport
- U.S. Permanent Resident Card
- Tribal Cards
Secondary IDs (2 forms if unable to provide Primary ID)
- Out of State Driver’s License
- In-state, Fully Valid Non-driver ID
- Out-of-state ID
- Social Security Card
- Employee/Student ID
PHOTOS
The library does not offer photo services. One photo is required. No Smiling and no glasses. Please visit Walgreens, CVS, FedEx or Walmart for photo services.

2 separate payments are required:

APPLY FOR A CHILD UNDER 16
- Both parents or guardians must approve and be in attendance for passport appointment
- If one parent is not available, the non-applying parent must complete form DS-3053. This form must be notarized, and the identification that was presented to the notary must be copied, back and front, and included with the form.
- You cannot renew your child's passport using Form DS-82.
- Passports for children under age 16 are only valid for 5 years.
APPLY AS A 16- OR 17-YEAR-OLD
- Applicants who are 16-17 must apply in person using Form DS-11.
- Passports for applicants 16 and older are valid for 10 years.
- You must show one legal parent, or guardian is aware you are applying.
RENEWALS
THE LIBRARY DOES NOT ACCEPT PASSPORT RENEWALS FOR ADULTS 18 OR OVER (DS-82 form)
- Applicants can submit renewals through the mail if their old passport was issued less than 15 years ago. If the passport was issued more than 15 years ago, they need to fill out a new application form (DS-11). If applicant was 15 or younger at the time their previous passport was issued, it is considered a new application, not a renewal.
- Online Renewal: Eligible U.S. citizens and nationals who want routine service can now renew their passports online.
